Please completely the application and attach the following:

1. Photos of the facility (before and after rehabilitation if available).
2. Signed EZ Agreement Form.
3. The EZ Zone Map indicating the location of the facility.
4. A copy of a current business license for the business to be conducted at the facility.
5. A document to prove ownership of facility (deed).
6. Proof of residency of owner of the facility.
7. If any investment has been made, proof of expenditure.
