Please complete Enterprise Zone Application form: - Photos of the facility (before and after rehabilitation if available).
- Signed EZ Agreement Form.
- The EZ Zone Map indicating the location of the facility.
- A copy of a current business license for the business to be conducted at the facility.
- A document to prove ownership of facility (deed).
- Proof of residency of owner of the facility.
- If any investment has been made, proof of expenditure.
Upon receipt of the application, EZ staff members will contact you. If you are not contacted within two days, please call the office at 340-773-6499 or 774-8104. |
